We went straight to IVF so not sure about TI/IUI coverage. COVERED at 100%: 4 retrievals, unlimited transfers, embryo freeze/thaw ICSI, mock transfers, diagnostics, labs, ultrasounds. COPAY - consults are $15 copay. Meds are $5 copay but must be ordered through Freedom Pharmacy, which means sometimes you wind up going out of pocket if you need to get a Rx filled immediately. NOT COVERED: Embryo storage, sperm storage, PGT-A. Embryo banking is not covered - you must use all euploid embryos or embryos above a certain grade (a subjective standard set by the insurance medical director) before they will cover another retrieval, unless you get an exception. Im assuming the exception would be fore something like pre-chemotherapy IVF. IMPORTANT - you must receive approval before you start a stim cycle, which can take up to 15 business days. Their rules on how often you need to get CD3 labs updated are confusing so to be safe just get them done again before you start treatment, and updated at least 1x a year To be covered over age 40, you must establish that you are DOR by age 39.
