No proof required for IVF (aka social infertility is covered) and no # of IUIs need to be done prior to starting IVF for full insurance coverage. This plan also fully covers PGT testing, embryo storage, and I believe donor sperm/egg costs. IUI is fully covered and any associated costs are not pulled from the lifetime max as long as its in network and there is a separate bucket for medication - I believe its $10k or $20k? Overall, fantastic coverage. You do need to sign up for UHCs fertility benefits program before you start.
